Even when students decided on a topic, they may still have a hard time getting down to the actual writing because their heads at this stage are full of unconnected ideas. Therefore, the best way of structuring them and thus achieving a perfectly organized paper is brainstorming. The thing that makes it highly effective is the fact that you actually know how to organize essential ideas in a manner that the reader can understand. Apart from being a framework that helps make the paper content stand out, an essay structure is also a right way of reaching out to your audience.

Most students do not know what a proper structure of an essay looks like, which is why their writing skills leave a lot to be desired. The major problem they usually face is related to having too much poorly structured information in their papers. Which is why learning how to structure it can help them organize their ideas efficiently and make their essay easy to follow. Being able to come up with a proper structure can also be a solution to your instructor’s challenging requirements.

Essay Paragraphs – the Structure

Nearly all students are aware of the so-called ‘five-paragraph structure,’ which is the first thing their tutors introduce them to when teaching the basics of essay writing. Though being a basic structure, it is rarely used in colleges or universities where all papers commonly comprise three paragraphs, i.e., the introduction, body, and conclusion.

Each student should know that the basic essay structure is the same for all essay types, be it persuasive, narrative or descriptive papers. A poorly structured essay will, therefore, hardly impress your instructor and you may even end up losing valuable grades. Good essay papers should read like a remarkable story and have a proper introduction, main body and conclusion. This example of a good standard structure:

  • Introduction ( 1 paragraph).
  • The main body ( any number of paragraphs depending on the required number of pages, i.e., three or more sections).
  • Conclusion (1 paragraph).

Essay Introduction

Being the first paragraph of your essay, the introduction lets the reader know about its topic, as well as your viewpoint regarding it. The opening should contain the thesis statement which can be two to three sentences long, comprising a summary of the main points and/or arguments presented in the essay. The manner in which you write your introduction will help the reader decide whether they would like to proceed with reading the paper or just stop right there. Therefore, knowing the type of audience you are writing for is vital because that way you will be able to come up with a compelling thesis statement that will grab their attention immediately.

Essay Body Paragraphs Structure

If the student chooses to go with the five-paragraph structure, they need to make sure that there are three body paragraphs in their paper. If it is a college or university student who has been assigned to come up with an essay containing five or more pages, then the number of paragraphs can be correspondingly higher.

The paragraphs between the introduction and conclusion are what is commonly referred to as the main body of the essay. You are supposed to make sure that you divide your main body into structural segments, such as subheadings and paragraphs so that it does not look so clustered and confusing to the reader. To achieve this, one should keep in mind a few simple rules, such as:

  • Each subheading and paragraph should present a new point, topic, subject, or idea.
  • Your argument should be supported by evidence, e.g., examples, data, facts, etc.
  • A quick conclusion needs to be drawn.
  • This part should keep the reader interested, so make sure that you do not drag one point for too long and include quotes wherever applicable.
  • And remember – you can always seek essay help to ensure that your main content is structured correctly and well-presented.

The second paragraph of an essay is where you make a smooth transition from the topic introduced in the first body paragraph, thus making it easier for the reader to follow your key ideas. The seamless transition concept is also applicable to all your remaining paragraphs; however, each point or idea should be supported with evidence, e.g., examples, research results or statistical data. Keep in mind that it is not merely your writing skills that your professor is testing, but rather logical thinking, as well as your ability to create a meaningful and persuasive argument that will help you draw a definitive conclusion.

Essay Conclusion Paragraph

The conclusion is the last paragraph of the essay, which provides the reader with a summary of the main points presented in your work. You shouldn’t introduce any new ideas here but rather restate the thesis statement in such a way as to avoid repetition and to induce boredom in your audience. A well-written conclusion will leave the reader with the best impression of your paper, so do not try to rush things up because you may end up ruining your entire essay. It is a good idea to come up with a rough draft of the last paragraph, which will let you iron out any remaining inconsistencies and repetitions in the final product.

The structure of an essay is an effective way of communication with the reader, showing how well the student understands the topic and requirements of the assignment, as well as their ability to raise an argument and reach a conclusion.

The citation or bibliography is an integral part of an essay structure that comes after the conclusion and states all the material that you have referenced. If your paper has direct quotes, paraphrased quotes or content from other sources, citations are essential and should, therefore, not be left out. Keep in mind that unless you add proper citations, you can get penalized for plagiarism or even lose grades for submitting a poorly referenced essay. If a specific format is stated in your instructor’s requirements, you need to make sure that you painstakingly adhere to it because failure to do so may entail very unpleasant consequences. In case no format has been specified, just pick any commonly accepted one that you find easy-to-use and format your paper accordingly.

How you structure your essay determines the manner (or sequence) in which you would like the audience to receive information contained therein. Depending on an essay type, the format of the body paragraphs may differ; however, the overall layout remains the same at all times. Essay planning and structure

It is really important to plan your essay before you begin writing. Planning will save you time later. It is also essential that you have a starting point to plan from, even if it is in a very rough form.

The obvious place to start is at the assignment question itself. From the question you can develop your answer in the form of a thesis statement . From there you can decide what your essay’s subtopics will be and what you want to say about them. After you have a basic idea of what you want to talk about, you can begin to write the essay.

However, when writing an essay, it can also be difficult to come up with a point of view early on. Therefore, instead of developing a thesis statement first, you may choose to read up on the assignment question and make notes on relevant concepts, theories, and studies. Once you have these notes and can develop a summary of the issues, it should be much easier to write a thesis statement.

Essay structure

All essays share the same basic structure, although they may differ in content and style. The essence of an essay is an opinion, expressed as a thesis statement or proposition, and a logical sequence of arguments and information organised in support of the proposition.

Tamer Gong Ye Huang is good at animals’ language. He can talk with every kind of animals. Hundreds of years ago, he overheard the sound from the white parrot and got a Tao book. He was enlightened and created Tamer after decades of cultivation. Tamer improve their own ability by domesticating different kinds of wild animals.

As the best hunter, Tamer can use bow and crossbow to attack the targets. Besides, Tamer enjoys the reputation of wild survivor. They are able to track the enemy or set the trap to cause damages or catch the enemy. There has been an original relationship between Tame and beasts. Animals can be loyal guards after being tamed by Tamers.

There is going to be a bit of stumbling though, so we thought we’d put together a beginner’s guide to show you the ropes, how the ropes work, and why you definitely shouldn’t pull that rope. Seriously, leave that rope alone.

Combined with our review diary, which you can read right here , you should be able to get to grips with the game pretty darn quickly. And if you don’t, well, we’re sorry, we tried.

Characters

After the big blustery opening you’ll be able to choose a character class. This is the most important decision you’re going to make in the game, and if you’re planning on playing with friends it’s worth having a chat about which roles people are going to take.

If you’ve played an MMO before then the character classes are going to be pretty familiar. If you haven’t, then here’s a brief break down of what you can expect from each of them.

Warrior

Warriors are your basic melee damage dealers. They should be front and centre in any scrap, soaking up the hurt to get more angry and more smashy.

Ranger

Probably the best class for solo play. They’re not as squishy as some of the magic-wielding classes when it comes to close combat, but they’re most effective controlling the crowd and dealing the hurt from as far away as possible.

Mage

Are of effect specialists. Where the ranger is more attuned to focusing on a couple of targets, Mages are all about keeping the crowd at bay and hurting many, many things with swirling blasts of magical magic.

Blood Knight

A new class for this sequel. Blood Knights are all about sacrifice. They deal huge damage, but at a cost to their own health. Best used as part of a group, with warriors soaking up the hits to free them up for explosive violence.

Monk

The Monk balances violence and healing powers. It’s another good bet for solo play, but works brilliantly when utilised properly in a group. Keeping other characters alive is a must deeper into the game.

The basics

Once you’ve chosen which class you’d like to play, you can pick one of two starting areas. They both end up in the same place, so only worry about picking one over the other if you’re starting your game with some friends.

The game does a pretty good job of walking you through what everything does, but we’ve got a few suggestions to make everything run that little bit better.

One good suggestion is to turn off tap to move in the settings. You’re probably going to be tapping on a lot of other characters accidentally in the scrum of the opening areas, and when you tap to deselect them, you’ll end up jogging off if you haven’t killed the functionality.

Patience is also a virtue to begin with. There are likely to be a lot of people running around trying to complete the same quests as you, so try not to become enraged when someone snaffles that quest item before you.

Quests

Keeping track of your quests can be a bit fiddly, especially if you’re trying to pay attention to the slightly haphazard story broiling around all the collect-this and take-that-there quests.

Checking your Quest Book regularly will keep you on track, and if you’re bored of following one quest line, you can jump onto any others that you have active.

Area quests pop up from time to time as well, and you start these automatically while you’re in that specific space. Your progress gets remembered if you wander away as well.

Taking side quests is a good way to build up your XP and stay ahead of the difficulty curve. They’re time consuming, but if you’re in a group you’ll blast through them pretty quickly.

Look at everything

There’s a dizzying amount of menus and gewgaws in Order & Chaos Online 2, but it’s worth checking them all regularly, because in the opening hours you’ll be given an awful lot of free stuff.

Jumping into dream dungeons is a good way to earn extra loot, and they shouldn’t be too tough to start off with either if you know what you’re doing.

You also get daily rewards just for logging in. So even if you’re not interested in having a play, quickly diving into the game will bag you some loot and keep your combo going for tomorrow.

And check the daily quests as well. Some of them are as simple as opening a mailbox or opening one of the chests you can grab after a certain time.

It can get a bit confusing, but just trawl through the menus, tap everything that says collect, and you’re half way there.

Other stuff

Your biggest problem at the start of the game is going to be your bag filling up. Make sure you sell as much trash as you can whenever you’re near a vendor. You’re going to collect a lot of worthless junk, and plenty of pieces of armour and equipment you can’t use too.

Remember to upgrade your weapon as often as you can to free up space as well. Once you get your special weapon after completing the first run of quests you’ll be able to fuse things to it to make it more powerful.

Crafting new armour is always sensible too. You’ll need to be near a crafting table to do it, which are usually near the merchants, but you’ll get better armour than the stuff you pull out of the innards of deceased pigs and spiders.

The opening section of the game isn’t exactly the most awe inspiring, but if you get your head down and plough through you’ll start unlocking more of the interesting things that Order & Chaos 2 has to offer.

bI. What is a MONK?

A monk is the barbarian class of Talisman Online, they carry either a shovel which looks like an oversized spear and a staff, which looks like a polearm.

A. Why should you choose a MONK as a class?

It’s simple, you want to be the captain of dungeon raids, a monk almost always leads the way in most dungeons. If you like the role of tanker and a stunner then a MONK is a class for you.

b        B. Strengths and Weaknesses
Strengths — The monk is the best stunner in the game, overall he has around 3 major stun skills up your arsenal. The stun time range from 1 to 5 seconds, that time could mean the difference of life and death. Aside from being the best stunner, the monk is also the best tanker in the game, his primary attribute is his Vitality followed by his Strength. He also has high defense, and a Monk’s equipment will always have more physical defense in them, more than any other classes. The monk ranks 1st in tanking, stunning and surviving.

Weaknesses — The monk deals low damage than any other class on monsters especially in dungeon raids, because of this, monsters tend to go for the healer or the one dealing most of the damage which is likely the wizard (however one skill of a monk draws the attention of all nearby enemies, more on that later). The monk has low magic defense (that’s why upgrading armors is a good thing). Some stunning skills also take a while to cooldown. The monk is basically melee, meaning he makes most of his attacks on close range. The monk also relies on fairies on boss fights (like any other class).

b        C. Monk vs. Other Classes

With high HP monks, a monk is virtually unstoppable most of the time. Given the right skills to use and perfect time could result of your opponent not being able to move at all on duels. Monk has a great advantage over assassins because Monks are melee and so as an assassin. He would have trouble dealing with the other 3 classes because they are all long range but given the right skills to cast, he can get close to them.

bII. Builds, Skills and Talismans
b                A. Tanker Build

This is the most common build of a Monk, the higher the defense and HP, the better. Tankers may lack damage but it is compensated by good stunning skills and the ability to better tank than others avoiding a team catastrophe.

For tankers, there’s no better weapon than a shovel. Simply because it can stun. When you stun an opponent for seconds, your giving yourself more time to tank, because you are not receiving any damage at all. The best assistant talismans for the job are the Buddha bone which is the talisman you’ll get at level 10. Then diamond sutra which is the first assistant talisman you’ll get (from a quest). Buddha bone has the important tanking skills, Mercy and Sacrifice. Mercy makes your enemies hate you more with every attack you deal and sacrifice grabs all enemies around you and will forcibly attack you instead of your party members for a limited time. Do upgrade these skills of the Buddha bone, specially sacrifice.

The diamond sutra is the best talisman a monk could have. The stun from Buddha Quake will stun enemies around you for 5 seconds (Pwnage!). And the Buddha shield skill will make a certain number of damage of every attack of an enemy be absorbed for a limited time (the damage absorbed depends on level and if all damage is absorbed, the damage it deals to you will be 1). On pressure situations, the Buddha shield can help you survive or buy a little time to get healed by your fairy teammates.

Oh by the way, the tanker build also needs a third talisman which you can get at level 20, the Treasure Loop. The treasure loop has one very useful skill “Robust”. Which increases your HP, better than a fairy’s sun needle does. But if combined with sun needle, your HP will sky rocket. So how can you have 3 talismans at once? Well since Treasure Loop will buff you for 30 minutes with Robust, you can replace it back with Buddha bone. Training your treasure loop so you can upgrade Robust to level 5 will also be useful but you need many energy points so you can go upgrade your Buddha bone again.

The last talisman is useless to the Tanker build, but you may play around with it. The Heart Lotus will give you the skills “Heart Fire” which increases your damage with Buddha and fire, and the super lame “Heart Lotus”, which summons a flower and that slows down enemies around it (which lures them as well) and makes invisible objects visible… (that part I haven’t been able to verify).

Important skills to upgrade on shovel: Improve Giant Attack (when you max it out, you don’t need to many stamina and you’ll get more break points).

Skills to upgrade on assistant talisman: All skills in Buddha bone and diamond sutra, Treasure Loop’s Robust spell.

Optional: Shovel’s improve giant halt damage, improve giant burst

Training: Buff with MERCY, and ROBUST. Attack with Giant attack first and get your first break point then stun it with Giant Halt. Continue attacking with Giant attack and if it’s enough, cast Giant Burst. When doing bosses, replace Giant burst with Giant Break if the boss is stun-able. Cast Buddha Shield when HP is getting low. Cast Sacrifice when enemy chases other members (this is tricky because sacrifice can also draw mobs in or if the enemy is too far, you might cast it late).

b                B. Power Build

A monk has two talismans to choose from, but if you want full power for your whole party, you might choose the staff (but I still recommend the shovel, im not a big fan of staff). The staff does melee damage as well as shovel, it also has a skill that lowers the enemies defense greatly (a great plus). Unlike the shovel, which deals Buddha damage (Buddha is actually earth), the staff deals more of the other elements like fire.

I you would play safe on a power build, you would still use the talisman “Buddha Bone”. But if you want to go all Power, the talisman “Heart Lotus” can give you an instant boost with Heart fire as it increases damage. The Diamond Sutra is always a must for it is offensive and defensive at the same time.

Important skills to upgrade on shovel: Improve Giant Attack Giant Burst (improving it at max, will deal more damage).

Important skills to upgrade on assistant talisman: Heart Lotus’ Heart Fire and Diamond Sutra’s Buddha Quake and Buddha Shield

Optional: Shovel’s Giant break can lessen your enemies Buddha defense, couple it up with Improved Buddha damage and heart fire. If you are into critical damage, you can improve the CS critical and damage.

bAPPENDIX

iThis area of the guide is highly controversial, as you can see on the replies to this guide, many have different views on the talisman weapons. This is purely my opinion and based on my experience

TANKING – This is quite probably the most controversial answer: my choice is shovel. Yes, the staff has high block but the shovel does stun more than a staff does. Shovel works well specially when bosses try to hit your allies, you can just do a burst attack that transfers it back to you (more times than not). The staff’s block is cool and all but surviving is your fairy’s job inside dungeons. However, in many cases like in BOSSES that can’t be stunned, staff does better.

GRINDING – I’m not sure at this one so I’ll just call it a tie. The shovel kills faster with the right combos but you take hits more and you have to sit down. The staff hits fairly good and you don’t have to sit down once in a while because of your blocking ability.

DUELS – Shovel, definitely, and there’s only one reason: “stun stun stun”.

POWER – Let’s call this another tie. Why? Yeah the shovel hits the most damage but it will take some time (and stamina) to even hit that high. The staff hits fairly but it doesn’t have burst. When you think about it, the shovel only looks powerful by the damage it does but the staff can outdo it by adding all the damage the staff has done over the course of time the shovel is getting burst points.

PARTY VALUE – Definitely the staff. The skill that lowers defense is a winner, which basically makes the party hit harder on mobs. If you just do grinding, staff is good when your with other players but once in bosses, it will be a different story.
isome might react again, heres my reason: if you lower the defense of the enemy, your partymates will hit harder, more times than not, will hit harder than you hit. So the boss will go after them than you.

The United States is a nation built on immigrants and this is why the nation was deemed “The melting pot,” because of all the different nationalities residing in this country. The argument of immigration and whether or not it enhances or threatens life in the United States is an on-going issue. Although we are trying to protect our citizens, immigration laws treat immigrants poorly.

This country needs to re-examine the immigration laws, so that they do not treat immigrants unfairly as well as protect our citizens while protecting the borders. Immigration has been responsible for cultural change and population growth throughout the country. The political, economic, and social aspects of immigration have created much controversy in regards to ethnicity, job security, job growth, economic benefits, where people live, not to mention impact on the amount of criminal activity, and work habits.

Reinforcing the borders, establishing clear immigration and deportation policies and doing something with those who are already illegally here but are law abiding citizens should be the goal for a new immigration policy. Instead of bickering on what should be done, congressional and senate leaders need to get the job done and come up with a flexible and workable reform to immigration. Illegal aliens currently in the United States should be eligible for legal status in eight years or some form of amnesty, as long as they learned English, had not committed a crime and paid their taxes.

The aim should not be to deport a record number of people. Spouses and minor children of a person who have gained legal status should be eligible to join their legalized family member. No one should be separated from their family. The majority of undocumented workers in the United States are currently holding low-wage, less desirable jobs where they risk injury without health coverage and often work in terrible conditions. Due to their illegal status, the government is failing to protect these workers from morally wrong employers.

Congress needs to eliminate the current system entirely or commit to making a fair program with strong worker protections that are vigorously enforced. In states like California, almost half of the students starting school are immigrants or the children of immigrants. As a result of immigration driven population growth, most public schools exceed their capacity. All children have the right to a good education. To alleviate overcrowding, schools use portable classrooms, and even hold classes in temporary instructional space, such as cafeterias and gyms.

One possible solution is to pass in many school districts is year-round school schedules. The aim with the year-round schools is to have the buildings in use during the summer months, with students on various schedules in order to maximize resources. There are millions of illegal aliens living in the United States. They are working jobs that need to be done, that someone has to do and that many of us would never take. They are still waiting for a change in the system which could regulate their status. Ignoring the problem will only make things worse. These people deserve fair treatment.

She demonstrates inappropriate behaviours such as seeking attention from her male peers, verbally abusive to staff and is prone to unpredictable extreme bouts of anger. She is academically able with an interest in performing arts.

    Strategies used:

    • Daily mentoring sessions to discuss issues arising from the previous day or evening. These sessions focus on developing positive relationships, appropriate boundaries and consequences
    • A daily behaviour report
    • Work with attendance officer focused on improving punctuality
    • Referral to school health officer
    • Involvement in craft group and dance lessons

    Outcomes and suggestions for the future:

    Chayenne now has more positive interactions with male students. Her punctuality and behaviour are much improved, specifically more control over emotional outbursts. Relationships with staff and peers are improved and she is making more academic progress. Chayenne will stay in regular contact with her mentor to discuss emotional anxieties. She will be more involved in leadership opportunities within school and referred to the counsellor. She will continue to develop her interest in performing arts.

    Case study 3

    Background and issues affecting student:

    *Lewis is a year 7 student with issues around attendance identified in year 6. There is a family history of involvement with social services. Lewis is on the special needs register at school for social, emotional and behaviour difficulties and for a specific learning difficulty.

    Strategies used:

    • Met with Lewis on the first day of term to discuss attendance report card (a system that rewards attendance)
    • Lewis was signposted to summer holiday activities to help with home circumstances over the long break
    • Inclusion into the primary transition group to assist in forming friendships
    • Allocation of a ‘buddy’ to Lewis through the peer listening scheme
    • Weekly meetings with the learning mentor to review how the year was progressing

    Outcomes and Suggestions for the Future

    Lewis has responded extremely well to the systems set in place. Attendance is still an issue although it is greatly improved. He is gaining confidence within the new academic setting and is improving his attainment. LMs will support Lewis on a medium to long-term basis as many of his issues revolve around the family and home. He will be supported and encouraged to take responsibility for himself and his education. Regular contact with his mentor has helped Lewis to remain focused and his improved attendance means that he is performing better in school which, in turn, is helping his confidence.

    Accelerating Best Care at Baylor Dallas

    Abstract

    A culture of quality improvement (QI) is needed to bridge the gap between possible STEEEP™ (safe, timely, effective, efficient, equitable, and patient-centered) care and actual usual care. Baylor Health Care System (BHCS) developed Accelerating Best Care at Baylor (ABC Baylor), an innovative educational program that teaches health care leaders the theory and techniques of rapid-cycle QI. Course participants learn general principles of continuous QI, as well as health care–specific QI techniques, and finish the course by designing and implementing their own QI project. ABC Baylor has been employed in a variety of settings and has spread its success to other organizations, especially small and rural hospitals. These hospitals, like BHCS, have demonstrated sustained improvements that are due in part to the use of ABC Baylor and its reliance on specific modules that focus on health care safety, service, equity, and chronic disease management. The role of ABC Baylor training and consulting is part of the overall culture and infrastructure that have allowed BHCS to achieve success in its improvement journey, including the receipt of several national awards and the achievement of high reliability in compliance with Centers for Medicare and Medicaid Services core measures of processes of care related to heart failure, acute myocardial infarction, community-acquired pneumonia, and surgical care. The culture of rapid-cycle QI facilitated by ABC Baylor serves to link BHCS’s vision and goals to practical execution.

    In 1999, the Institute of Medicine (IOM) released its landmark report, To Err Is Human, which estimated that there are close to 90,000 annual preventable deaths in the United States secondary to flaws in health care delivery processes ( 1 ). The IOM’s second report in 2001, Crossing the Quality Chasm, defined high-quality health care as care that is safe, timely, effective, efficient, equitable, and patient-centered (STEEEP™) ( 2 ). In accord with these reports, the health care industry has recognized the need to improve the safety and reliability of health care. Moreover, the concept of “value-driven health care” recently emerged to reflect the need for transparency in pricing, transparency in quality, and demonstrable efforts to improve quality and safety ( 3 ). Both the public and the payers are holding the health care industry accountable.

    Across many industries, the term “high reliability” refers to a low rate of product defects ( 4 , 5 ). For example, Ireson has defined reliability as “the capability of the product to perform the specified function in the designated environment for a minimum length of time or a minimum number of cycles or events” ( 5 ). While the definition of a product defect may be relatively straightforward in an industry such as electronics, it is less clear how to define defective care in the health care industry. One example of a defect in care may be a case in which a physician misses the opportunity to recommend a mammogram to an eligible patient. The Centers for Medicare and Medicaid Services (CMS) has defined defective health care as the failure of a hospital to comply with 24 recommended, evidence-based processes of care related to heart failure, acute myocardial infarction, community-acquired pneumonia, and surgical care. These best practices are collectively referred to as core measures ( 6 ). In addition, some professional organizations such as the Society of Thoracic Surgeons have defined recommended processes of care for each surgical intervention ( 7 ). Improving the reliability of health care across core measures means increasing the proportion of patients who receive all recommended best practices.

    A culture of quality improvement (QI) is needed to improve the reliability of health care and thereby bridge the gap between possible STEEEP care and actual usual care ( 2 , 8 , 9 ). By training staff in QI techniques and adopting an organizational focus on health care improvement, organizations have improved all dimensions of STEEEP measures, including equality ( 10 ), efficiency ( 11 ), patient-centeredness ( 12 ), and safety and clinical effectiveness as defined by CMS core measures ( 13 ) and health care process measures for cardiovascular illness ( 14 , 15 ) and pneumonia ( 16 ). But despite the success of QI programs and their use in health care since at least 1995 ( 17 ), hospitals vary in their ability to train staff, implement the programs, and sustain health care improvement as an organizational focus ( 18 , 19 ). Many of these differences arise from factors such as organizational leadership, hospital culture, hospital size, and inertia ( 20 , 21 ); moreover, small and rural hospitals may lack the resources to implement QI programs ( 22 – 25 ). These are differences that must be overcome for successful adoption of QI programs. Disseminating information and lessons learned from successful QI programs is one way to help hospitals overcome barriers to implementation ( 13 , 26 , 27 ).

    This article describes a successful health care QI training program and its implementation and sustainability in a large, integrated health care system.

    ABC BAYLOR: THE PROGRAM

    Setting and origins: Baylor Health Care System

    Baylor Health Care System (BHCS) is an integrated health care delivery system that comprises 20 owned, leased, or affiliated hospitals; 5 short-stay hospitals; more than 80 primary care, specialty care, and senior health centers; 12 rehabilitation clinics; 18 ambulatory surgery centers; more than 400 physicians employed by the HealthTexas Provider Network; 3000 other BHCS-affiliated physicians; and the Baylor Research Institute.

    In September 2000, the BHCS Board of Trustees passed a resolution on QI:

    Whereas, maintaining the status quo or achieving quality and safety levels only equal to or slightly better than national, regional, or local norms is not compatible with the BHCS Vision and Mission Statements; … and Whereas, regulatory and legislative changes and a growing number of more informed patients support better quality patient care and safety; … Therefore, be it resolved, that the Board of Trustees of Baylor Health Care System hereby challenges itself and everyone involved in providing health care throughout the system to give patient safety and continuous improvement in the quality of patient care the highest priority in the planning, budgeting and execution of all activities in order to achieve significant, demonstrable and measurable positive improvement in the quality of patient care and safety.

    BHCS affirmed its commitment to this resolution by creating the Institute for Health Care Research and Improvement and recruiting the appropriate talent to lead its efforts. In 2001, the Best Care Committee was formed to oversee the planning, budgeting, execution, and reporting of activities to improve the quality and safety of care throughout BHCS. Health care improvement leadership continued to be developed through exposure to multiple resources, such as participation in the Institute for Healthcare Improvement Breakthrough Series projects ( 28 ), as well as the training of 30 physician, nursing, and administrative leaders in the Intermountain Healthcare Advanced Training Program in Health Care Delivery Improvement ( 29 ).

    Inspired and supported by Intermountain Healthcare, BHCS developed and launched its own training program, “Accelerating Best Care at Baylor” (ABC Baylor) ( 30 ), in 2003. ABC Baylor, an innovative educational program focused on health care QI, teaches health care leaders the theory and techniques of rapid-cycle QI, outcomes management, and staff development. It facilitates the enhancement of skills needed by physicians, nurses, administrators, and others to lead QI efforts ( 27 , 30 ).

    Although numerous QI programs are available through external consultants, BHCS decided to develop internal resources to train its workforce in techniques of continuous quality improvement (CQI) rather than outsource the training to external entities. This decision was based on the recognition that a more reliable health care delivery organization needs to incorporate CQI as one of its core competencies and needs to develop the necessary talent internally. This decision also allowed BHCS to standardize the language and tools used for CQI.

    Differences between ABC Baylor and other QI training programs include the integration of the main messages of the IOM, including the need for STEEEP health care ( 2 ); reliance on QI examples that include clinical care; simplicity of the material and exclusion of advanced statistical concepts; and individual modules that focus on health care safety, service, equity, and chronic disease management.

    ABC Baylor training objectives and curriculum

    ABC Baylor’s first objective is to have a cultural impact on participants by creating a sense of urgency around the quality chasm described by the IOM ( 2 ) and around the need to adopt health care improvement methods. Next, ABC Baylor aims to provide experiential training in tools of health care improvement using a variety of sources such as classic CQI ( 4 ), total quality management ( 31 ), Lean ( 32 ), Six Sigma ( 33 ), and Toyota production systems ( 34 ). Results of a participant satisfaction survey, as well as informal comments from both faculty and participants, suggest that the amalgamation of the two previous objectives results in a synergistic compounding benefit beyond the benefit derived from non–health care-specific training programs in QI techniques.

    The ABC curriculum contains two major components:

    • Specific principles of CQI that cover team building, feedback skills, process modeling, data system design, Pareto techniques, statistical process control, and cost-quality relations
    • Clinical improvement modules that cover customer service skills, safety, equity, and chronic care

    Five ABC courses target different members of the health care community. The four-session core course, which is aimed at change leaders (physicians and nursing and administrative leaders), entails didactic instruction and design, implementation, and evaluation of a QI initiative. The course introduces administrative and clinical leaders to QI methods and ways to measure and analyze evaluations; helps them to understand the relationship between quality and cost; and provides a general understanding of statistical variation, the tools of patient safety, and leadership strategies for QI.

    In Session 1, course participants learn about the urgency for QI in health care, improvement tools, evidence-based care, basic statistics, and methods for improved communication. Sessions 2 and 3 focus on different modules of health care QI, including patient safety, service excellence, health care equity, and improvement of chronic care. The modules include a general lecture, a discussion of the current state of BHCS with regard to the module focus, identification of improvement opportunities, and examples of QI projects. In Session 4, course participants present QI projects of their own design ( Table ). To date, 1316 clinical and administrative leaders have completed the ABC Baylor course, becoming QI champions and leading projects in their own environments.

    Other ABC Baylor programs are also offered:

    • “ABC Fast Track,” a 1-day course aimed at frontline caregivers that provides training for participants to support quality initiatives introduced by change leaders
    • A half-day “Introduction to Quality Improvement” program, which focuses on practicing physicians and involves a physician group retreat
    • A version targeting residents in training in several BHCS residency programs, which consists of three half-day sessions held 6 weeks apart, augmented by coaching received from residency directors and including projects completed by participating residents
    • Shorter customized versions of the core course delivered for special settings, including nursing homes and home care

    The targeted audience for ABC Baylor includes both internal BHCS teams and external customers. Internal teams can be geographic (e.g., from a certain hospital), functional (e.g., financial officers from diverse BHCS entities), or clinical process-based interdisciplinary teams (e.g., the pneumonia team, the acute myocardial infarction team). External customers consist of health systems, hospitals, physician groups, and health maintenance organizations using any combination of services ranging from full best care operationalization to specific training programs. Modules can be detached from the ABC Baylor course and delivered independently for specific customers.

    ABC Baylor has grown rapidly since its implementation in 2001, partly because of accountability structures imposed by institutional goals cascaded down to all enterprise units. Rapid growth has also been stimulated by the BHCS pay-for-performance program, which links executive compensation to hospital and system success in achieving increased compliance with CMS core measures and has increased the sense of urgency for health care QI ( 35 ).

    Program success and dissemination to other health care settings

    ABC Baylor has been studied and implemented in a variety of settings and has spread its success to other organizations, especially small and rural hospitals. The core course was incorporated in a BHCS-led Agency for Healthcare Research–funded randomized controlled trial of health information technology and QI education on quality of care in 47 rural Texas hospitals ( 22 , 25 ). The study examined ways to translate CQI education to small rural hospitals.

    In 2006, BHCS entered into collaboration with Jefferson Medical College of Thomas Jefferson University in Pennsylvania to conduct a QI demonstration project that provided training in rapid-cycle QI techniques to select Pennsylvania community hospitals. Participants successfully implemented a variety of QI projects, showing that CQI programs developed by large health care systems can be adapted and applied successfully in other settings, especially smaller rural and community hospitals that lack the necessary resources to establish such programs independently ( 27 ).

    Hazleton General Hospital was one of two rural area community hospitals selected to participate in Accelerating Best Care in Pennsylvania. Five projects were undertaken for the demonstration: provision of heart failure discharge instructions to eligible patients, surgical antibiotic prophylaxis, care of the stroke patient, administration of an antibiotic within 4 hours to pneumonia patients, and pneumococcal vaccine administration to eligible patients. From January 2007 to June 2008, compliance with these measures rose from 79% to 93% for provision of heart failure discharge instructions; 70% to 96% for timely antibiotic administration for emergency department patients with pneumonia; 85% to 92% for pneumococcal vaccination of inpatients who met the criteria; 13% to 100% for computed tomography scans within 20 minutes for stroke patients presenting to the emergency department; and 20% to 83% for prophylactic antibiotics given to patients within an hour of particular types of surgery ( 36 ). These improvements suggest that the principles taught in ABC Baylor can lead to improvements in health care quality in community hospitals ( 13 ).

    In addition, BHCS’s QI efforts, including ABC Baylor, have led to substantial recent recognition. BHCS was the 2008 recipient of the National Quality Foundation National Quality in Healthcare Award (which is awarded annually to a single health care organization to recognize the organization for its “proactive and exemplary response to the national call for QI and accountability”) and the 2007 recipient of the Leapfrog Patient-Centered Care Award (granted to the hospital or health system whose board has most successfully driven the creation of a true partnership between patients and their caregivers). BHCS also ranked third among 73 health care systems across the United States in performance on publicly reported clinical quality measures, including CMS core measures ( 37 ).

    One example of an ABC Baylor project that led to substantial QI is the reduction of prolonged intubation after cardiac surgery at one of BHCS’s hospitals, Baylor Medical Center at Irving. The Society of Thoracic Surgeons identifies prolonged intubation beyond 24 hours after surgery as a defect in health care ( 7 ). The interdisciplinary improvement team at Baylor Medical Center at Irving recognized the lack of an extubation protocol as a cause of this defect and developed a standard protocol. Results of the application of this protocol are shown in the Figure . Prolonged intubation has not occurred for the last several months.

    DISCUSSION

    BHCS has been widely successful in its improvement journey, reaching high reliability in the use of core measures ( 37 ) and winning several national awards. ABC Baylor training and consulting is only a part of the overall culture and infrastructure responsible for the successes of BHCS.

    The QI principles taught in ABC Baylor have been successfully disseminated to and implemented in a variety of settings, including academic teaching hospitals, community hospitals, and small rural hospitals. The program’s success in improving reliability across health care settings has been due to its use of examples that include clinical care; the simplicity of the material and its exclusion of advanced statistical concepts; and the modules that focus on health care safety, service, equity, and chronic disease management. Thus, the program is accessible to a variety of health care employees.

    As BHCS has successfully adopted useful dashboards addressing various clinical quality indicators, ABC Baylor serves as an essential set of tools targeting the improvement priorities identified by the dashboards. The culture of rapid-cycle QI facilitated by ABC Baylor serves to link BHCS’s vision and goals to practical execution.

        High DPS means nothing if you can’t land your hits.

        So mobility > dps.

        Well it’s pretty relative to the player playing the class, if they aren’t using the proper combos or glyphs, or bad with mana, dodging or blocking their dps output will not be good.

         

        However based on pure stats like each classes attack speed, damage ranges and critical damage the max dps for the classes is set in stone. Zerker then slayer, warrior, lancer for melee. However warrior is being buffed this month so that may or may not change.

        Your just repeating what i have said in all my post in this thread lol.

        check first post for dps tests, first time I saw this post was on NA forums and i recall all the characters had around the same quality of gear. I have not played any character over 20 so I can only comment on what I have seen so far and in these videos slayer is 2.5 faster than a zerk, I doubt zerk dps will surpass slayer that much if at all, but my last judgment will be when I have the chance to check it out myself as I do not have the knowledge  to know  how much biased the video is:)

        Also yes zerk does have a higher weapon rating but also slower attack speed and ultimately dps is damage divided by  time.

         So you tell me I can’t read and then go on to repeat what someone else stated after I had already stated it and in the reply to my post also repeat something I said, that being warriors getting a buff. Some people these days.

         

        I was just merely poiting out your post makes it seem as if peoples dps rates will be set in stone, so I expanded and mentioned why it will be so varying.

        Yep, I’m going slayer because I like the class and enjoy the fact that you have to think on your feet.

        Yes its easy to spam with this class…but like others are saying its all about movement.  In fact TERA is all about:

         

        1.) Efficient Movement

        2.) Skill timing.

        3.) Aim

        Currently waiting for the MMO industry to put out something good.

        Berserkers.

        Berserker is the first Melee DPS class I will cover. Berserker’s wield giant axes, wear heavy armor and have the highest weapon damage rating; tied with an Archer. Their damage rating is second only to Archer. Their crit rate is highest; tied with Slayers. Berserkers for some time were mistaken as a third possible tank, due to the fact that they also wear plate armor. This however is certainly not the case, although if pressed a Berserker can tank, it is not a viable options. Berserkers are meant to attack from behind along with the other DPS classes. Despite their heavy armor, they can be blown up pretty quickly whether it’s by a monster or by another player.

        Berserkers are the slowest Melee DPS class, with an attack speed of 0.7 which is matched only by Sorcerer. Despite this slow attack speed, Berserkers can crank out a great deal of damage, with their high damage and weapon damage ratings. Their range is only a short three meters, but this is offset by the splash damage effect of their skills, and the spin of their charge-up technique.

        Berserkers have become notorious for their ability to one-shot almost every other class, which obviously appeals greatly to many players. Berserkers are quite possibly one of very few classes that can give a Lancer a difficult time, seeing as their heavy armor mitigates much of a Lancer’s already poor DPS, as well as their ability to hit extremely hard.

        The splash damage of a Berserker is slightly less than the damage dealt by a full-on hit, but is still absolutely devastating if it crits. As an Archer I personally fought against a Berserker in a 5v5 fight, and was one-shot by the splash damage of a Berserker. This damage hit me for 16,000 which is four thousand HP over what my health would have been if I were at full health. Those who were in the direct path of his hit and were also crit were hit for nearly 20,000 if not higher depending on their armor type.

         

        Slayer.

        Slayers are the second and last Melee DPS class of TERA. Slayer’s wield giant swords, wear light armor, and have the third highest damage rating. They are tied for highest critical hit rate with Berserkers. They also have the second-highest weapon damage rating! Slayers were also for some time thought to be a viable option for tanking in TERA, but lack of a proper taunt means they must out-damage the rest of their group while also attempting to dodge the majority of incoming attacks. Due to the complexity of tanking as a Slayer, and their squishiness, they are not a viable option as a tank. As with Berserker and the Ranged DPS classes, they are meant to flank their enemies and attack from behind or the sides where they will hopefully not be hit.

        Slayer’s are in the mid-range of attack speed, a 0.6 which they are tied with Priest and Mystic for. While they are rather squishy if caught, they have greater mobility than a Berserker. With self-buffs for run speed, coupled with a ranged sword strike; they are a worthy opponent for the Ranged DPS classes, in some cases considered the counter class for Archers and Sorcerers.

        Slayer’s damage out-put is slightly lower than that of a Berserker, but their faster attack speed, ranged attack, and movement speed buffs more than make up for it. While a great majority of Berserkers can be kited in circles, Slayers are extremely difficult to outrange, seeing as their ranged attack has a range of fourteen meters; this is only one meter less than the range of a Archer or Sorcerer’s base attacks, and four meters longer than the range of a Priest or Mystic.

        Slayers are said to be the closest to an Assassin or Rogue as you will get. Despite their lack of dual wield, their feel, damage, and the way they play are more similar to an Assassin than any other class.

         

        Warrior..

        Warrior is the second and last tank in TERA. Warrior’s wield duel swords, wear light armor, and have the highest HP pool and movement speed out of all eight classes. Furthermore, Warrior attacks are an arc – meaning they are not purely single-target characters. If you and your buddy are in the path of the swinging swords, you will both get hit, instead of just one of you. Warrior’s damage rating is higher than a Lancer, making them third lowest; they are positioned between Sorcerer (lower) and Slayer (higher) on the damage rating.

        Warriors are classified as the evasion tanks of TERA. They have fewer taunt options than a Lancer, but their higher DPS, as well as an AoE stun, make them a viable option as a tank. Warriors are a similar play style to rogues and assassins in other MMO’s, but lack the stealth feature. Stealth is a skill that the TERA developers did not want to add to the game for any of the classes, as it is too game-breaking in PvP.

        Warriors have a great technique for getting away or catching up to someone, although I personally do not believe it is as good as a Lancer’s. Warrior’s dodge is a great way to get away or chase someone; the simple flaw with this is the animation is different for each race. While a Castanic Female Warrior’s dodge is a fancy leap (which can leap over attacks), other races must suffice with a roll or somersault. Because of this, the majority of Warriors are generally Female Castanics; the smaller hitbox of the female Castanic also plays a role in this, as Warriors are a tank that shouldn’t be getting hit by what they are fighting.

         

        Lancer..

        Lancers are the primary tanks in TERA, though as with primary tanks in most games, they are far and few between. They wield a large shield and lance, wear heavy armor, and have the second highest HP pool out of all eight classes. Their damage rating is the third lowest (Priest and Mystic having the lowest damage rating), but this is off-set by their survivability. And of course, they have an AoE taunt.

        Lancers are not only sturdy, but one of their skills blocks 100% of all damage taken from the front and, partially, the sides. In general, DPS classes will attempt to skirt around behind the Lancer, but a good Lancer can simply re-position his or her shield block. Lancers also have a self-heal, which obviously can be a life saver.

        This class also has an excellent technique for getting away (or catching up), which is their forward-charging dash. This skill seems to have a high knock-down rate when it hits, but it is also worth mentioning that this skill causes the Lancer to run faster than mounted players as well. So if someone mounts up to try and get away, the Lancer can chase them down and knock them off their horse. Alternatively, they can use it to get away, or to catch up to ranged classes.

         

        Archer..

        Archers are what I like to call the snipers of TERA. Their play style is very reminiscent of snipers, seeing as in PvP and boss fights they tend to snipe the primary target with their charge up shots; Penetrating Arrow and Radiant Arrow. As is typical of Archer classes, Archers wield a bow, and wear light armor. But here is where Archers differ from the norm; they do not require ammo. Archers in TERA have unlimited ammunition, which is explained by the developers as the arrows technically being “magic”. Due to this, bows are classified as magical weapons when it comes to crafting.

        Archers have the highest damage rating, weapon damage rating, and range (fifteen meters), but have the second lowest HP pool of all eight classes. Despite their low HP pool, they are not quite as fragile as other ranged/support classes due to the fact they wear light armor, and not cloth. Their use of light armor, as well as their dodge skills increases their survivability greatly; but as always in TERA it is up to the player to make proper use of these defenses.

        It is worth mentioning that the 15 meter range applies only to their base attack (left click by default). Skills such as Penetrating Arrow have a range of 17-19 meters, and can have their range increased by Glyphs. At maximum, Penetrating Arrow’s range can be increased to roughly 21 meters with Glyphs. That being said, the farther away you are the less damage you deal.

        Archers are a single-target DPS class, although they do have a minor Lock-On AoE. In later levels they will have one or two more AoE options, but the majority of their skills are single-target. That being said, their initial charge up skill that they acquire (Penetrating Arrow) can damage multiple targets if they are bunched closely together or are stacked on top of each other. Thus the “Penetrating” in the skill name. Penetrating Arrow is the first of two charge up skills that Archer gets, the second being Radiant Arrow, which is nearly identical but is purely single-target unlike its predecessor. Due to being purely single-target, Radiant Arrow is capable of putting out even more damage than Penetrating Arrow, but its range is slightly shorter as well to compensate.

        As in other MMO’s, Archers in TERA have a variety of Traps. Originally, there were four different trap skills available to Archer. However, with the incredible strength of Poison Arrow on their side already, Archer’s poison trap was removed. The first trap an Archer gets is called Cobweb Trap. As you might imagine, this trap is an AoE snare. The second trap that an Archer acquires is called Incendiary Trap. This trap is pretty simple; it does AoE damage, and that’s about it. Our third and last trap, is Concussion Trap; quite possibly my favorite. Concussion Trap is an AoE stun, and with the use of glyphs, the duration of the stun can be increased by 40%.

        Archers are a very mobile class due to their ranged nature. Albeit squishy, their ability to kite, dodge, and their high DPS makes them a force to be reckoned with on the battlefield. An Archer’s role both in PvE and PvP are to focus the main target; generally whatever the tank is occupying. Due to their single-target nature, they are very useful for quickly blowing up a single target. Coupled with their debuff (Mark of Velik) which causes the Archer to deal 10% more damage to the marked target, and Sniper’s Eye which increases strength by 50, and adds an additional 25% damage in PvP (though decreases attack speed by 30%) they can increase their damage against the main target by a significant amount.

        If there is a Lancer in the Archer’s group, the Lancer can use one of his skills to further decrease the target’s defense. This lethal combination can result in the Archer critical hitting PvP targets for all of their HP. Warrior also has a similar skill known as Combative Strike, which is acquired at LV6.

        And of course with Close Quarters (melee-range AoE stun), Backstep (dodge), and Breakaway Bolt (dodge with damage), Archers have a wonderful slew of abilities they can use to outrange or escape their foes.

        Yap I had read this class description before as well just did not know who posted it. Anyway I doubt weapon damage rating etc dictate anything, for one they are slower. Just look at aion’s gladi, they have the strongest weapon damage (polearms) yet their dps is way below of a sin  and even below a ranger that knows that they are doing (single target wise).

      Now we’ll focus on more practical applications of Intimidation. Traning dummies make good targets, but they don’t hit back. The golems at the edge of this terrace, however, generate attacks for you to block. Axe Block, that is.

      While using Intimidation, a successful Axe Block allows you to use Axe Counter. Not only will you deliver a strong attack in response, but while doing so your resistance to knockdown and stagger increases, and any incoming damage is reduced by 50%.

      Excellent work. Timing is everything in combat. Intimidation also increases the amount of damage you can stop with an Axe Block, as well as extending its protective effects to allies behind you.
